Information for Returning Student-Athletes

To ensure that you will be eligible to participate in your respective pre-season, please download, complete and return the following forms:

Please fill out each portion of the forms completely and to the best of your knowledge. If these forms are not on file prior to the first pre-season practice, you will be ineligible to participate in pre-season practices.  A link to pdf versions of these forms can be found on the right side of this page. About the Forms

Health Questionnaire Forms

Medical history questionnaires are a requirement for all student-athletes in order to participate in varsity athletics at Johnson State College. Complete the medical history form. Pre-participation physicals are not required beyond your first year unless: you have had a major injury or illness since your last physical, or you have taken a year off from varsity athletics in between seasons.

Returning student-athletes will not be allowed to participate in pre-season practices unless all pertinent paperwork is on file with the Athletic Training Office.

All insurance and waiver forms must all be signed and completed.

All athletes must fill out the insurance form and emergency contact information in its entirety. Be sure to have the insurance policy holder sign the form.  All other forms must be signed by the student-athlete, and any student athletes under the age of 18 years old must have a parent or guardian signature in the space provided on each of the forms sent by the Athletic Training Staff.

IMPORTANT NOTE

All returning student-athletes who took a year off from NCAA Athletics must have a new physical completed. The NCAA is now mandating that all physicals be completed by a physician.  We can no longer use nurse practitioners, or other related practitioners to do our sports physicals.
