Student Health Form

Vermont state law requires that students be up to date with immunizations prior to starting the semester.

  • You will not be able to submit your housing contract until we receive a completed Vermont State Colleges Student Health Form, so please do the following as soon as possible:
  • Download and print out the form. Click here to download the form.
  • Visit your health care provider to get the health form completed and signed.
  • Return the completed form to JSC. Please submit the form to:
