What is This Form For?
The Family Educational Rights and Privacy Act (FERPA) is the federal law that governs release of and access to student education records.
This Information Release Form enables you to list individuals (parent, guardian, etc.) whom you wish to allow access to general information from your educational record.

You can submit the form when you check-in at your New Student Registration session, or you may send it to the JSC Dean of Students Office, 337 College Hill, Johnson, VT 05656.
